I give consent for Cate to work with me using Rapid Eye Technology as well as the multiple other healing modalities she is trained in.
I understand she is not a licensed LCSW therapist.
I understand if I report self harm or harm to another, Cate must report it according to Utah law.
I understand that any form of inappropriate physical, emotional, verbal, sexual or psychological violence or acts towards Cate will result in immediate termination of the session with no refund and any remaining sessions are forfeited.
Insurance is not accepted as payment for services.
We reserve the right to refuse service to anyone.
24 hours’ notice is required to reschedule an appointment.
For the first missed appointment, 100% of the session fee is charged, as well as for the second appointment, and thereafter 100% of the session fee is charged.
There is no refund for payment after payment is made.
All packages expire 3 months after the date of purchase.
